Farmers Student Athlete Citizen Award

Consideration Criteria for the Farmers Student Athlete Citizen Award

• The student must be a full-time high school student.
• The student must attend a school which is a member of the WIAA or OSAA.
• Both male and female candidates are eligible.
• Achievements in both individual and team sports will be considered.
• Candidates are required to carry a GPA of 3.0 or greater (on a 4.0 scale).
• Extracurricular involvement, including community service, both on and off campus, must be demonstrated and detailed on nomination form.
• Extracurricular activities/community involvement will have a 40% weight, academic accomplishments will have 30% weight and athletic accomplishments will have 30% weight in the selection process.
• All submissions must include three photos of the nominee that represent their academic, athletic and community involvement for use in the FSN commercial (Photos will be returned).

Nominating a Farmers Student Athlete Citizen:

• Complete a Farmers Student Athlete Citizen Award nomination form. Forms are available at fsninsider.com.
• Submit form and three photos to FSN Northwest, Attn: Farmers Student Athlete Citizen Award via mail to: 3626 156th Ave SE, Bellevue, WA 98006 or by fax at 425-641-9811.
• A representative from FSN Northwest will contact the nominator if the student has been selected as an award recipient.

Award Recognition:

• Award recipients will be featured in a 30-second commercial on FSN that will run on the network throughout the 2009-10 school year.
• At the end of the school year, those students named as a Farmers Student Athlete Citizen will receive an award certificate, a copy of the commercial they were featured in and a letter of recommendation for use in college applications.

Applications will be accepted from November 1, 2009 through May 1, 2010. Award recipients will be notified of the award starting December 15, 2009 with the last one announced on in June 2010.
